Boley Centers Homeless Supportive Housing Program, Butterfly Apartments
Permanent, supported independent apartments for individuals who are homeless and who have mental illness, who may also have co-occurring substance abuse problems. Supportive housing staff provides in-home and in-community services, skills teaching and supports that the individual needs to live independently. Supportive housing staff ensures the individual's access to all needed psychiatric, medical, dental and community supports and services to assist their recovery. Independent living residences for disabled adults. Program will arrange for transportation.
Hours: 8:30 a.m.–5:00 p.m., Monday-Friday

Intake/Application Process: Agency referral required. Referral to be made to the Admissions Department of Boley. Items to bring to intake: Picture ID, Social Security card, Proof of Residency, Proof of Axis I Mental Health diagnosis, any and all historical information.
